This is an online e log book to discuss our patient de-identified health data shared after taking his / her / guardians signed informed consent. Here we discuss our individual patients problems through series of inputs from available global online community of experts with an aim to solve those patients clinical problem with collective current best evident based input.

A 58 year old male patient labourer by occupation , 2 years back he came to hospital with cheif complaint of bilateral pedal edema ,shortness of breath ,backpain which was diagnosed as renal failure and initiated dialysis weekly twice.
History of present illness:-
Patient was apparently asymptomatic 2 years back then ne noticed pedal edema.
Now patient has edema of left upper limb from 2 months.
Patient complains of cough. 
Patient complains of back pain.
No h/o fever.

Past history:-
Patient is known case of hypertension since 4 years.
Patient is not a known case of diabetes. 

Personal history:-
Patient has normal eating habit with mixed diet.
Apetite is normal.
Sleep normal.
Bowel and bladder movements are normal.
Urine output is normal.

Family history:-
No significant family history. 

General examination:-
Patient is conscious ,coherent,cooperative lying in supine position on the bed in well lit room.
Moderately built.
No pallor,cyanosis,clubbing of fingers,icterus.
Vitals:-
Temperature:Afebrile
Pulse rate:82 per min
Respiratory rate:24 /mm/min
Spo2:98
Grbs:126mg%

Systemic examination:-

CVS:-
S1 S2 heard.
Respiratory system:-
No dyspnea
No wheeze
Abdomen:-
Shape is scaphoid.
No tenderness.
CNS:-
Patient is conscious. 
Speech normal.

Investigations:-

  Renal function tests:-Serum iron:-
83 ug/dl(Normal-61-157)
HIV 1/2 Rapid test:-
Non reactive.
HBsAg Rapid:-
Non reactive.
ECG:-
Provisional diagnosis:- 
CKD on MHD.

Questions:
Was the edema of upper limb due to venous hypertension?
