This is an online e log book to discuss our patient de-identified health data shared after taking his / her / guardians signed informed consent. Here we discuss our individual patients problems through series of inputs from available global online community of experts with an aim to solve those patients clinical problem with collective current best evident based input.
A 17 year old male patient who is a student came to opd with cheif complaints of itchy ring lesions on arms,abdomen,thigh and striae over the abdomen, thighs,upper limbs since 8 months.
History of present illness:-
Patient was apparently asymptomatic 8 months back then he developed ring lesions on arms,abdomen,thighs and he also started developing striae over abdomen ,thighs,upper limbs .He is having facial puffiness since 3 months and also pedal edema since 3 months.
Past History:-
Patient is known case of hypertension since 6 months under regular medication.
Patient is not a known case of diabetes,tuberculosis.
No h/o surgeries done.
Personal history:-
Patient has normal eating habit with mixed diet.
He is not an alcoholic,smoker.
Apetite normal.
Sleep normal.
Bowel and bladder movements normal.
Family history:-
No significant family history.
General examination:-
Patient is conscious ,coherent ,cooperative .
No pallor
Pedal edema is seen.
No cyanosis,clubbing,icterus.
Vitals:
Temperature-Afebrile.
Pulse rate -78 beats/min.
Respiratory rate-17cycles/min.
BP-160/100 mm/hg.
Systemic examination:-
CVS-S1,S2 heard.
Respiratory system-No dyspnea,no wheeze.
CNS:-He is conscious.Speech is normal.No abnormalities detected.
Abdomen-Purple straie seen on abdomen
Investigations:-
T3,T4,TSH:-
Provisional diagnosis:-?Iatrogenic Cushing's syndrome and Tinea corporis .
